एक न्यायिक पैनल ने यह तय किया है कि इंदौर में पानी दूषित होने से 30 से ज़्यादा लोगों की मौत रोकी जा सकती थी। पैनल ने संस्थागत विफलता और व्यवस्थित खामियों का हवाला दिया है। रिपोर्ट में सीवरेज नेटवर्क की गलत योजना और क्रियान्वयन पर प्रकाश डाला गया है, जिससे पीने के पानी में सीवेज मिल गया। 859 दिनों से ज़्यादा की देरी वाले महत्वपूर्ण टेंडर प्रक्रियाओं में देरी को भी एक बड़ा कारण बताया गया है। पैनल ने इन देरी के लिए कुछ अधिकारियों को मुख्य रूप से जिम्मेदार ठहराया है, जबकि दो IAS अधिकारियों को बरी कर दिया है।
