The loco pilot of the Sikkim Mahananda Express, which derailed at Bihar's Ara Junction on September 7, had insufficient route learning. Preliminary investigations found the crew responsible for overshooting a red signal and then reversing the train, causing the derailment. The Prayagraj Division suspended officials for deploying the crew despite incomplete training, violating railway rules and a recent Railway Board warning. This occurred despite the board's explicit instruction for strict action against such violations.
